Next Step for Youth Who Need More Support
Fresh Coast Counseling partners with therapists, schools, prescribers,
medical providers, hospitals, crisis teams, case managers, and community
professionals to support children and adolescents who may require more structure and intensity than routine outpatient treatment.
When to Consider Day Treatment
Day treatment may be worth considering when a youth's emotional, behavioral, developmental, or mental health needs are significantly interfering with fuctioning across home, school, peer relationships, or the community - and routine outpatient services are not providing enough structure or support.
Symptoms or behaviors are affecting multiple areas of daily life
The youth needs frequent opportunities to learn and practice skills
Weekly outpatient treatment is not providing enough support
Family and provider coordination needs have become more complex
School participation or functioning is significantly declining
The youth is transitioning from hospitalization or another higher level of care
Emotional regulation, safety, or behavior concerns are escalating
A structured communicty-based alternative may help prevent further deterioration
These indicators do not automatically establish eligibility or admission. Fresh Coast Counseling completes an individualized clinical screening that considers current needs, functioning, safety, developmental fit, available supports, treatment history, program capacity, and the youth's likelihood of benefiting from day treatment.
Collaborative Care,
Not Isolation
Fresh Coast Counseling is designed to work alongside the youth's existing treatment and support network. With appropriate authorization, our team coordinates with families, outpatient providers, schools, prescribers, medical professionals, case managers, and other involved supports to promote consistency across settings.
Outpatient Providers
When appropriate and authorized, Fresh Coast may communicate with existing therapists and other treatment providers about goals, progress, safety concerns, transitions, and continuity of care.
A referral to Fresh Coast does not have to mean starting over.
When appropriate, our goal is to build on existing relationships, treatment progress, and supports while providing the additional structure of day treatment.
Schools
The treatment team may collaborate with school professionals to understand functioning, coordinate supports, and help prepare for successful participation in the school environment.
Prescribers & Medical Providers
With appropriate authorization, Fresh Coast may coordinate relevant medication, health, functioning, and treatment information with involved medical and prescribing professionals.
Families & Community Supports
Parents and legal representatives remain central partners, and other community providers or case management supports may participate when appropriate.
What Helps Us Review a Referral
A referral does not need to be perfect or include every possible record. The most helpful referrals give our team a clear picture of the youth's current needs, functioning, safety, strengths, existing supports, and why a higher level of care is being considered.
Health and Medication Needs
Include current medications when known and any emergency medication, medical device, allergy, or health action plan that may affect participation.
Current Concerns
Briefly describe what has changed, what is prompting the referral now, and where the youth is having the most difficulty.
Current Functioning
Share how the youth is functioning at home, at school, with peers, and in the community.
Safety Information
Include known recent concerns such as sucicidal thoughts, self-harm, aggression, elopement, hospitalization, crisis involvement, or access to weapons.
Additional records may be requested when needed and when appropriate authorization is available. Examples may include recent assessments, discharge summaries, treatment plans, medication information, school records, safety plans, or other clinically relevant documentation.
Identify outpatient therapy, psychiatry, school supports, case management, recent hospitalization, or other services already involved.

Share the youth's strengths, interests, important relationships, and what the youth, family, or referral source hopes to gain from treatment.
1. Referral Review
Our team reviews the referral for basic program fit, current concerns, safety information, and missing information.
4. Clinical Screening
An authorized clinician reviews the youth’s current needs, functioning, safety, supports, and level-of-care considerations.
What Happens After You Refer
After a referral is received, the Fresh Coast team reviews the available information, contacts the appropriate person, gathers additional information when needed, and determines the next step in the screening process.
Submitting a referral does not guarantee an intake appointment or admission. Referral decisions are based on individualized clinical screening, safety, developmental fit, available supports, and program capacity.
2. Follow-Up Contact
We contact the parent, legal representative, referral source, or other appropriate person to clarify needs and next steps.
5. Next Steps
The referral may move forward to intake, require additional information or supports, be temporarily deferred, placed on an operational waitlist, or be referred to another level or type of care.
3. Additional Information
When needed and authorized, we may request records or communicate with current providers, schools, prescribers, or other involved supports.
6. Ongoing Communication
When appropriate and authorized, Fresh Coast communicates next steps with the family and referral partner so expectations remain clear.
